Caregiver Support, Independent Living and Community Integration, Post-18 Services and Transition Support
School to Community
Rainbow Centre
Problem Statement
A significant number of clients with severe disabilities are unable to access post-graduation services due to their inability to meet eligibility criteria, incompatible services, and/or long waiting lists.
Solution
The project seeks to:
Understand the needs and life goals of persons with disabilities and facilitate access to relevant social services or community resources such as volunteers;
Provide individualised care planning to support the transition of persons with severe disabilities from school to community;
Foster a community network of care to continually engage clients to prevent regression and provide support services; and
Develop a toolkit to enable the social service offices to better support the clients.
Outcomes
Persons with disabilities and their families are able to access social services and community resources, enabling them to continue living meaningful and engaged lives in the community after graduation.
