Problem Statement

Stroke survivors with disabilities and their caregivers may face challenges adjusting to life after discharge from hospital. Beyond physical rehabilitation, they often require additional support to manage the emotional, social, and practical aspects of recovery, as well as to develop the skills and confidence to navigate life at home and in the community.

Solution

This Integrative Health Coaching Post-Stroke programme develops and conducts a holistic, individualised, and strength-based health coaching intervention that seeks to support first-time stroke survivors with disabilities and their caregivers during the transition from hospital to home. Through personalised coaching and peer support, the programme aims to strengthen self-management skills, promote emotional wellbeing, and support recovery in community settings.

The programme comprises :

ARCH (‘Aspirational Rehabilitation Coaching for Holistic Health’): Equipping first-time stroke survivors with self-management skills to navigate recovery and daily life post-discharge, while reducing caregiver burden;
PEER (‘Peer Enriched Environment for Recovery’): Facilitates peer learning and support to strengthen self-management skills and encourage sustained recovery.

Outcomes

The project aims to support stroke survivors with disabilities and their caregivers by:

Enhancing emotional, mental, and physical wellbeing;
Strenghening self-management skills and confidence in navigating post-stroke recovery;
Fostering support networks and relationships through coaching and peer engagement ; and
Supporting continued recovery through community-based outpatient rehabilitation and support;
