Community Portal for Stroke Survivors and their Caregivers
Singapore National Stroke Association (SNSA)
Problem Statement
Stroke is one of the leading causes of adult physical disability in Singapore. While there are existing resources, services, and programmes to help stroke survivors, the information is scattered and resides mainly with individual service providers.
Solution
This portal serves as a community platform that provides stroke survivors and their caregivers with practical advice and professionally endorsed best practices in caring for themselves and their loved ones. It provides information on support services, resources available in the market and allows the exchange of experiences and expert perspective on stroke management.
Outcomes
Stroke survivors and their caregivers receive professional advice on caring for themselves and their loved ones, gain access to information on support services and resources, and build peer support networks.
